STUDENTS AND PARTICIPANTS WAIVER AND RELEASE FORM
Welcome Storm Academy!
Before you or your child participates in any academy program, we require that this waiver and release form be completed and signed to acknowledge the risks associated with sports activities and agree to the terms herein.
WAIVER AND RELEASE:
As a participant, or the legal parent or guardian authorizing a minor participant, I hereby give consent to participate in sports activities conducted by Storm Academy (a.k.a "STORM SPORTS CLUB AND FACILITIES MANAGEMENT SOLE PROPRIETORSHIP LLC"). I recognize that there are inherent risks associated with sports and related activities, and I agree to assume full responsibility for any personal injury sustained in connection with participation in such activities at Storm Academy.
I hereby release, waive, discharge, and covenant not to sue Storm Academy, its officers, instructors, agents, and employees (collectively known as "Releasees") from any liabilities, claims, demands, or causes of action that may arise from participation in sports activities.
I am aware and understand that Storm Academy does not carry or maintain health, medical, or disability insurance for any participant. I agree that if medical treatment is required during participation in sports activities at Storm Academy, the cost of such treatment will be my sole responsibility.
I have read, and I fully understand the terms of this waiver and release form. I acknowledge that by agreeing to this document, substantial rights are being given up, including the right to sue for damages in the event of an injury, and I accept this waiver and release form voluntarily.
